Curogrid has two plans for clinics. Plus is ₹399 a month and Pro is ₹999 a month, for the whole clinic. Paying for the year costs 20% less: ₹3,830 for Plus and ₹9,590 for Pro. GST of 18% is added at checkout. #### Is there a free trial? Yes. Every new clinic gets a 5-day free trial of the Pro plan, the higher of Curogrid's two plans, so you see everything before you choose. #### What is the difference between Plus and Pro? Plus and Pro differ in their limits. Plus allows 1,000 new patients a month, 6 staff logins and one branch. Pro has unlimited patients, 12 staff logins, up to 5 branches, WhatsApp from your own number and priority support. Visits, prescriptions, billing and stock are the same on both. #### Is the price per doctor? No. Curogrid's price is for the whole clinic, however many doctors work there. The plan sets how many staff logins you have: 6 on Plus and 12 on Pro. #### Is GST included in the price? No. Curogrid's prices are shown before GST. It shows who is next, who has been called, who is with the doctor and how long each patient has waited. The doctor sees the same line on the phone and calls the next patient from there. One button calls the next token, one marks that the patient came in, and one marks the visit done. - **Wait times that change colour.** Every row shows how long that patient has waited. A wait of more than 30 minutes turns orange and a wait of more than an hour turns red, so nobody is forgotten at the back of a full room. The top of the screen shows the average wait for the day. - **Babies and senior citizens are marked.** A token can carry one of five tags: Emergency, Senior citizen (60+), Pregnant, Disability or Baby under 2. The tag shows beside the name at the desk and in the doctor's line. - **Stepped out is not lost.** A patient who has gone for tea or to the chemist moves to Stepped out and keeps their token. One tap brings them back into the line. - **Walk-ins and appointments together.** The day's appointments are listed beside the line, with who is late. When a booked patient arrives, one button puts them in the line with everyone else. #### Can the doctor call the next patient from the phone? Yes. The phone app's My line screen shows the tokens in order with each wait time, and a Call next button at the bottom. #### Does it work for a clinic that takes only walk-ins? Yes. Tokens do not need an appointment. The desk gives a token to whoever walks in, and that is the whole line. #### Can a patient be seen out of turn? Yes. Any waiting token has its own Call button, so the doctor or the desk can call someone ahead of the order when it is needed. ### Appointment scheduling for clinics Curogrid's appointment book is the clinic's shared calendar. It gives each doctor a column and a colour, in day, week or month view. A course of visits, such as six physiotherapy sessions or four antenatal check-ups, is booked in one go, and each booking shows its place in the course. Each doctor's bookings are a different colour, so a shared calendar still reads at a glance. - **Day, week and month.** Switch between the three views, or show one doctor alone. A line across the day marks the time now. - **A course of visits in one booking.** Turn on the course switch and Curogrid books several visits together, at the same time of day, a set number of days apart. Each one is labelled, for example “Session 3 of 6”. - **The wording follows the speciality.** A physiotherapy clinic books a course of therapy sessions, a skin clinic books laser and peel sessions, a gynaecologist books antenatal visits, a surgeon books dressings. The usual number and gap are filled in for each. - **Booked patients join the same line.** On the day, expected appointments are listed on the Today screen. When the patient arrives, one button gives them their place in the line. #### Can I book several visits for one patient at once? Yes. Turn on the course switch in the booking drawer, choose how many visits and how many days apart, and Curogrid books them all at the same time of day. #### Can each doctor have their own calendar? Yes. Every doctor has a column and a colour in the appointment book, and you can filter the book to one doctor. #### Can patients book online? Yes. An online booking page for patients is included in every plan. ### Patient records for clinics: one file for each patient In Curogrid every patient has one file, the clinic's electronic medical record for that person. Allergies, long-term conditions and current medicines are at the top. It is the first thing on the screen every time the file opens. - **Last visit, next visit, still to pay.** Three boxes under the patient's name answer the three questions asked at the desk: when did they last come, when are they due, and do they owe anything. - **A first card that follows your speciality.** A family doctor sees blood pressure over the last five visits. A paediatrician sees the child's weight. A diabetes clinic sees HbA1c. A physiotherapist sees the course of sessions. - **Nine tabs, one patient.** Overview, Visits, Prescriptions, Tests and scans, Appointments, Bills, Readings, Files and Consents. Nothing about the patient lives in a second place. - **Act from the file.** Give a token, book a visit or start a bill from the top of the file. The patient's number can be copied, or opened in WhatsApp, in one tap. #### Does every patient get an ID number? Yes. Each patient gets a number in your clinic's own series, shown beside their name and on their bills. You can search by it, by name or by mobile number. #### Can I keep reports and photographs in the file? Yes. The Files tab holds documents and photographs. Storage is 150 GB on the Plus plan and 200 GB on Pro. #### Are diagnoses coded? Yes. Diagnoses are searched from an ICD-10 list by name or by code, with the everyday word shown where it differs, for example “Chloasma [melasma]”. #### Does Curogrid warn about a patient's allergy? Yes. When a medicine on the prescription matches something the patient is recorded as allergic to, the doctor's screen shows a red line that says “Check before giving”. It is a reminder to check, and it does not block the prescription. ### Clinic billing that shows who still owes you Curogrid's billing screen is where a clinic raises bills and takes payment. It shows four numbers for the day: what is still to collect, what was taken (split by UPI, cash and card), what was billed, and bills started but not saved. Below them is every bill, with a Take payment button on each one that is not fully paid. Take payment records what came in today, and the rest stays against the patient's name. - **Who owes.** One tab lists every patient with money outstanding. The dashboard also flags a bill that has been unpaid for over 30 days. - **Day close.** At the end of the day, close it: one place to check what was billed and what was collected before the drawer is counted. - **Services priced once.** Your services and fees are set up once, starting from your speciality's list. A bill is then a few taps, and every bill has its own number in your series. #### Can I take part payment on a bill? Yes. A bill can be part paid. Curogrid keeps what is still to pay against the patient and lists it under Who owes. #### Does it record how the patient paid? Yes. Each payment is recorded as UPI, cash or card, and the day's total is shown split the same way. #### Can I print or export my bills? Yes. The billing screen has Print and Export buttons, and filters to narrow the list first. #### Is there a GST report? Yes. Reports has a GST report that shows tax by rate with the HSN and SAC summary, for the dates you choose. ### Also in every plan - **Visits and prescriptions.** Medicine names are offered as you type, from your speciality's list. Save a set of medicines you write together and reuse it. A medicine that matches the patient's recorded allergy is flagged in red. - **Tests and scans.** Order a test or a scan in one tap on the visit, or a procedure from a new order, and follow it step by step: Ordered, Sample taken, At the lab, Result in, Checked, Given to patient. - **Stock.** Keep count of what the clinic holds, including items that come in batches and expire, such as vaccines. - **Expenses and vendors.** Record what the clinic spends and which vendor it was paid to, next to what it earns. - **Reports.** 10 reports: Collections, Services and treatments, Doctor-wise, Outstanding dues, GST, Expenses and what was left, Patients, Visits and diagnoses, Appointments and waiting, Stock. Staff attendance and an activity log of who did what are kept as well. - **Staff and roles.** Each person signs in as themselves. No. The phone app is the same clinic and the same data as the web version of Curogrid, with no extra charge. A token given at the desk appears in the doctor's line on the phone. ### Which phones does it work on? The Curogrid phone app is made for Android phones and iPhones. The apps are being listed on Google Play and the App Store, and are not in the stores yet. Curogrid also runs in a web browser on any computer or tablet. ### Can the doctor write a prescription on the phone? Yes. Yes. The Plus plan at ₹399 a month is made for a solo or small clinic. It includes 6 staff logins, so the doctor, the compounder and the receptionist each sign in as themselves. #### My patients do not book. They walk in. Does it still work? Yes. The Today screen is built around walk-ins. The desk gives a token, and the line shows who is next and how long each person has waited. Appointments and walk-ins sit in the same line. #### Can I write a prescription quickly during a busy OPD? Medicine names are offered as you type, tests and scans are one tap, and the last visit's medicines can be repeated with one button. You can also keep a set of medicines you write together under a name and put it on the next patient's visit. #### Does it record blood pressure and sugar for long-term patients? Yes. Vitals are saved at each visit. The patient's file draws the last five blood pressure readings, and a Readings tab draws blood pressure, weight, pulse and SpO2 over every visit. Test results such as HbA1c are kept under Tests and scans. #### Is Curogrid an EMR for general physicians? It does that work. Each visit records vitals, complaint, examination, diagnosis, medicines, tests and advice. The diagnosis is searched by name or ICD-10 code from a list of 365 common OPD diagnoses, or written in your own words. The dose is always the doctor's to write. #### Does it calculate a child's dose from the weight? No. The weight is recorded at the visit and shown beside the prescription. Curogrid has no dose calculator and never suggests a dose. #### Can the receptionist give priority to a small baby? Yes. A token can be marked “Baby under 2” at the desk. You can change the number and the gap before you save. #### Can I sell a package of sessions? Yes. A “10 session package” is one of the starter services, at a starter price of ₹4,500. You can change the price, or add your own packages, and bill it like any other service. Curogrid does not count sessions down against a paid package. #### How do I know which session a patient is on? Every booking in a course carries its place, such as “Session 3 of 6”, and the patient's file opens on a Sessions card that shows how many are done, such as “3 of 6 done”, and when the next one is booked. #### What happens when a patient misses a session? The Sessions card in the patient's file says which session was missed and that it was not rebooked, so the course is not quietly left unfinished. The desk then books the missed session again. #### Can I record home visits? Yes. “Home visit” is a starter service, set at 60 minutes and a starter price of ₹1,200. To choose clinic management software, test it against your own clinic's day before you pay. Ask nine questions: who will type into it, how it handles walk-ins, whether it fits your speciality, how it books repeat visits, how it takes part payments, what it costs for the whole clinic, whether it works on a phone, where the records are kept, and how you get them out. A demo shows the software on its best day, with a tidy sample clinic. The problems appear in the second week, on a full morning, at your own front desk. Each question below is one you can check in the demo itself. #### 1. Who will actually type into it? Choose for the person at the front desk first. In a small clinic the receptionist, the compounder or the sister registers the patient, gives the token and takes the money, and the doctor types the least. If they find the software hard, it will be abandoned however much the doctor likes it. In the demo, ask to see the front desk's screen first, and picture your own staff using it on a full morning. #### 2. Does it handle walk-ins, or only appointments? You need a token and a waiting line, with appointments joining the same line when the patient arrives. Some software assumes every patient has booked, which does not suit an OPD where people walk in. Ask to see exactly how a walk-in is given a token, and time it. #### 3. Does it fit your speciality on day one? Ask to see the software as it looks for your speciality before anyone has configured it. A generic system gives every doctor the same blank screen and leaves the setup to you: your services, your medicines and your way of booking, typed in over several evenings. A physiotherapist should see sessions and packages on the first day. A paediatrician should see syrups and a growth record. #### 4. How does it treat a course of visits? Good software books a whole course in one step and shows where each patient stands in it. This matters if your patients come back several times, for physiotherapy sessions, dressings, laser sittings or antenatal check-ups. When each visit has to be booked one at a time, the next one is easily forgotten and the patient stops half way. #### 5. Can it take part payments and show who owes? The software should record each payment by how it was paid, and give you one list of everyone who still owes money. Patients pay part now and the rest later, and they pay by UPI, cash and card on the same day. Without that list, the dues live in someone's memory. #### 6. What does it cost for the whole clinic? Compare the total you would pay in a year. Some products charge per doctor, some per user and some per patient or per message. Ask for the price with your real number of doctors and staff, and ask what happens to it when you add a branch. #### 7. Does the doctor need a computer? Check that the phone app shows the line and the patient's file, on the same data as the desk. A doctor who consults with a phone on the table needs the whole record there. Some phone apps are a separate, smaller product that shows only part of it. #### 8. Where are the records kept, and who can see them? Ask where the servers are, whether each staff member has their own login, and whether you can limit what each role sees. A receptionist does not need clinical notes. Be cautious of any vendor that answers this question with only the word “secure”. #### 9. Can you get your records out? Ask now how you would export your patient list and your bills, because one day you may change software. If the answer is vague, treat it as a no. #### A checklist to take into the demo 1. Ask the vendor to give a walk-in a token while you time it. 2. Ask for a new, empty account set up for your speciality. 3. Book one patient who must come back five times. 4. Make a bill, take half of it by UPI, and find that patient on the list of who owes. 5. Open the same patient's file on a phone. 6. Sign in as a receptionist and see what is hidden from that login. 7. A token system gives every patient a number when they arrive and shows one order to the desk and the doctor at the same time. It works for walk-ins and booked patients alike. Its purpose is to let everyone see who is next, so the receptionist is not asked every two minutes and the doctor knows how many people are outside. #### What a token system has to do A token system has five jobs. If yours misses one, that is where the arguments at the desk come from. - Give every patient a number the moment they arrive, booked or not. - Show the order to the desk and to the doctor at the same time. - Show how long each person has waited, as well as their position. - Let you call someone out of turn when it is right to, without losing the order. - Keep the place of a patient who steps out for ten minutes. #### Walk-ins and appointments in one line Run one line for everybody. A booked patient is expected at a time and joins the line when they actually arrive, so everyone sees a single order. The usual mistake is to keep a diary for appointments and a pile of paper slips for walk-ins. The two then compete, and the patient with a 10:15 appointment argues with the walk-in who arrived at 9:40. #### Babies, senior citizens and urgent cases Mark priority patients at the desk and let the doctor decide the order. A baby with a fever or an elderly patient who cannot sit for an hour should be seen sooner than strict first-come-first-served allows. When the mark shows in the doctor's line, the doctor makes the call, and the receptionist is not left to judge it alone in front of a full room. #### The patient who steps out Treat “stepped out” as its own state: the token is kept, the line moves on, and the patient goes back in when they return. Someone always goes to the chemist, to the ATM or for tea. On paper they either lose their turn or are called three times to an empty chair. #### Measure the longest wait If you measure one thing, measure the wait of the person who has waited longest, and the average for the day. “Nine waiting” tells you little. “One patient has waited 34 minutes” tells you what to do next. A clinic that knows its average wait can decide whether it needs a second consulting room or a later start for appointments. #### When paper tokens stop being enough Paper tokens stop being enough when there are two doctors, when appointments and walk-ins mix, or when the doctor wants to see the line without walking to the door. Paper works for a small, steady clinic. Past that point, move the line onto a screen that the desk and the doctor share. #### How to start a token system in your clinic 1. Decide that every patient gets a token on arrival, including those with an appointment. 2. Decide who may be called out of turn, and write the list down for the desk. 3. Agree what happens to a patient who steps out, and for how long their place is kept. 4. Put the order where the desk and the doctor can both see it. 5. At the end of each day, note the longest wait and the average. #### How Curogrid does it Curogrid's Today screen is this system. The desk gives a token in a few seconds. Three boxes show who is next up, who has been called and who is with the doctor. Each waiting patient shows their wait, which turns orange when it is long, and the top of the screen shows the average wait for the day. Speciality-specific clinic software arrives already set up for one kind of clinic. Five things change with the speciality: the services and price list, the medicines offered as you type, the way a dose is written, how repeat visits are booked, and what the patient's file shows first. You can check all five in a five-minute demo. Software that says it works for “all specialities” often means it is equally generic for all of them: one screen, one price list you fill in yourself, one way to book. The five checks below tell the two kinds apart. #### 1. The services and the price list A specific system arrives with your services and a starting price for each, and lets you change them. An ENT clinic bills for ear syringing, nasal endoscopy and audiometry. A gynaecologist bills for antenatal visits, scans and IUCD insertion. If you have to type all of these in yourself, you have been given a blank form. #### 2. The medicines offered as you type The first names offered should be ones you write. A paediatrician writes syrups, drops and suspensions. A dermatologist writes creams, gels and shampoos. If a skin clinic types “ad” and is offered an adult antibiotic before adapalene gel, the list was written for someone else. Some clinics need no medicine list at all: a physiotherapy visit records the therapy given. #### 3. The way a dose is written The software should know your directions. “Twice a day after food” covers a tablet. It does not cover “in the right ear”, “thin layer on the affected area”, “wash off after five minutes” or “by mouth with the dropper”. These phrases are how specialists write, and software that offers them saves typing on every prescription. #### 4. How repeat visits are booked A course of visits should be one booking that then shows the patient's place in it, such as “Session 3 of 6”. This is the biggest difference between the two kinds of software. A physiotherapist books six sessions two days apart. A skin clinic books four laser sessions a month apart. A gynaecologist books antenatal visits through a pregnancy. #### 5. What you see first in the patient's file The first thing on the screen should be the thing your speciality watches. A family doctor wants blood pressure over the last few visits. A paediatrician wants the child's weight. A diabetes clinic wants HbA1c. A physiotherapist wants to know how many sessions are done. #### How to test a vendor's claim 1. Ask to see a new, empty account for your speciality, with nothing filled in by the vendor. 2. Open the price list. Are your services there? 3. Start a prescription and type two letters. Are the first suggestions ones you would write? 4. Book a patient who needs to come back five times. How many steps did it take? 5. Open a patient's file. None of these needs new equipment. A course is planned for a reason, and a patient who stops at session three has paid for part of a result. The clinic also loses the booked slots. A patient who has six dates in hand has agreed to six sessions. A patient who is told “come again on Thursday” has agreed to one, and has to decide again each time. Give the dates on paper or by message, so the family can see them too. #### Keep the same time and the same therapist Give the course one time of day and, where your staffing allows, one therapist. A fixed slot becomes part of the patient's week, and a therapist who saw the last session does not need the history repeated. If the slot must change, move all the remaining sessions in one go so the patient has one new time to remember. #### Show the patient where they are in the course Tell the patient their place at every visit: “this is session three of six”. A patient who can see the end of the course has a reason to reach it. Use the same count in the patient's file, so that anyone at the desk can answer “how many are left?” without calling the therapist out of a session. #### Follow up a missed session the same day Give a missed session a fixed routine, so that it is followed up every time and by the same steps. 1. Mark the session as missed when the slot has passed. Do not leave it sitting in the diary as if it were still to come. 2. Call or message the patient the same day. Ask only when they can come next. 3. Rebook the missed session, and move the later ones if the gap between sessions matters to the plan. 4. Write one line in the file on why it was missed. A patient who has missed twice for the same reason needs a different slot. 5. Once a week, list every patient with a missed session that was never rebooked, and call them. #### Agree the money before the first session Decide with the patient whether they pay for each session or for a package, and write down what was agreed. If you sell a package, state how many sessions it covers, how long it stays valid, and what happens to sessions that are not used. If the patient pays in parts, record each part on the day it is paid. A patient who is unsure what they owe may find it easier to stay away than to ask. #### What to count each month Count three things each month: courses started, courses finished, and missed sessions that were never rebooked. We have no sourced benchmark to offer for a normal completion rate, so compare your own months with each other. Manage antenatal visits as one course for each pregnancy. Book the coming visits before the patient leaves, follow every scan and test from the order to the result, keep a weekly list of who has missed a visit, and agree the fee at the start. The clinical schedule is the doctor's. The clinic's job is to make sure nothing on it is lost. This guide is about running the clinic. It does not say which visit, scan or test is due at which week. Follow the schedule you practise by. #### Treat each pregnancy as one course of visits Keep the antenatal visits of one pregnancy together, numbered, and apart from the patient's other visits. Then the file answers the two questions asked at every check-up: which visit is this, and was the last one attended. A diary that holds each visit as a separate, unrelated appointment cannot answer either without someone turning back the pages. #### Book the next visits before she leaves Book ahead at the desk, on the day of the visit, while the patient and her family are in front of you. Book as far ahead as your schedule is fixed. If the gap between visits changes later in the pregnancy, book the first part now and the next part when the doctor sets it. Keep the same weekday and time where you can, because the person who comes with her has to plan for it too. #### Follow every scan and test to its result A scan or test has four states, and the clinic should know which one each is in: 1. Ordered: the doctor has asked for it, and the date it is wanted by is written down. 2. Done: the patient has had it, at your clinic or outside. 3. Result back: the report is in the patient's file. 4. Seen: the doctor has read it and noted what follows. The gap that causes trouble is between the first and the third: an order was written and no report ever came back. Check the open orders before each antenatal clinic day, and ask for the missing reports when you remind the patient of her visit. #### Keep a weekly list of who has missed a visit Once a week, list every antenatal patient whose visit date has passed without her coming, and call each one. Give the list to one named person at the desk. Record the outcome of each call in the file: rebooked, delivered elsewhere, moved away, or not reachable. A missed visit with no note beside it looks the same next week, and gets missed again. #### Put the antenatal package in writing If you charge one fee for the pregnancy, write down what it covers and give the patient a copy. State these points: - Which visits are included, and whether extra visits are charged. - Which scans and tests are included, and which are billed apart. - Whether the delivery is part of the package or a separate bill. - How the fee is paid: at once or in parts, and when each part is due. - What is refunded if the patient moves to another doctor. Record each part payment on the day it is made, so the desk can tell her what is still to pay without asking the doctor. #### If your clinic does ultrasound A clinic that does obstetric ultrasound in India has registration and record-keeping duties under the PCPNDT Act and its rules. They are separate from your clinical record, and the forms and the periods for keeping them are set by law. We do not summarise them here. Check the current rules with the authority that registered your clinic or with your legal adviser, and name one person in the clinic who is responsible for the paperwork every day. #### Give pregnant patients their place in the line Decide a rule for the waiting room and tell the desk. A woman late in pregnancy should not sit for an hour behind routine follow-ups. Track a laser or peel package as two records that must always agree: the sessions, booked and counted as a course, and the money, billed once and paid in full or in parts. Write the terms down at the sale, book every session that day, and check each week that no patient has had more sessions than they have paid for. Packages go wrong in a few predictable places: a session that nobody counted, a balance that nobody asked for, and a patient who returns after eight months expecting the sessions that were left. Each has a fix at the desk. #### What to write down when you sell a package Put the terms on one page and give the patient a copy. A package sold on a spoken promise is remembered differently by each side. - The treatment and the area treated. - The number of sessions in the package. - The planned gap between sessions, as the doctor has set it. - The price, and whether it is paid at once or in parts. - How long the package stays valid. - What happens to a session the patient misses without notice. - Whether unused sessions are refunded or can be moved to another treatment. - That the doctor may change or stop the plan for clinical reasons, and what is charged if that happens. #### Book every session on the day of the sale Book the whole course before the patient leaves. Sessions that are weeks apart are easy to lose: with a long gap, neither the patient nor the desk has a reason to think about the next one until it is overdue. A patient who leaves with every date has a plan. Send a reminder before each session, because a date fixed a month ago is easily forgotten. #### Count sessions done against sessions paid For every open package, the desk should be able to say three numbers without searching: sessions in the package, sessions done, and the amount still to pay. Count a session as done only when it has been given, on the day, by the person who gave it. Do not write a fresh bill each time the patient pays. One bill with part payments shows what is still to pay at a glance. A fresh bill for every payment makes one package look like several small sales. Decide your rule, for example that the balance is cleared before the last session, and print it on the terms. #### Photographs and consent Take written consent before the first photograph and before the first session. Before-and-after photographs are how a skin clinic shows progress, and a photograph of a patient is personal data. Keep the consent for the clinical record separate from any consent to show the photographs to other people or to use them in publicity, and let the patient agree to the first without the second. Check the current data protection rules before you use a patient's photograph outside the file. #### When a patient stops in the middle 1. Call after the first missed session, and rebook it. 2. If the patient wants to stop, note the reason in the file and the date. 3. Apply the refund or transfer rule that was written on the terms. 4. Close the package, so it no longer appears among the open ones. 5. Record a child's weight and height at every visit, measured the same way each time, and record each vaccine on the day it is given with its name, batch and date. Keep both in the child's one file. Then any visit, for any reason, can answer the two questions a parent asks most: is the child growing, and what is due next. This guide covers the record and the running of the clinic. It does not reproduce a vaccination schedule or say what a normal measurement is. Use the schedule and the growth charts you practise by. #### What to record at a growth check Record the same few things at every visit, including visits for a fever or a cough. A growth record is only useful as a series, and a series has gaps if measuring is kept for “growth visits” alone. - The date of the visit. - Weight, on the same scale each time, with the same amount of clothing. - Height or length, measured the same way each time. - Any other measurement your practice takes for the child's age. - Who measured, when more than one person does it. #### Measure before the child reaches the doctor Make measuring a job for the sister or the compounder, done while the family waits. The doctor then opens the file with today's figures already beside the earlier ones, and the whole consultation goes to the child. 1. Give the token at the desk. 2. Call the child to the scale in token order, ahead of their turn with the doctor. 3. Enter the weight and height in the child's file straight away. Do not write them on a slip to be copied later. 4. Send the family back to the waiting area until their token is called. #### What to record at a vaccination visit Record enough that another doctor, years later, can tell exactly what was given. The visit charge is your fee and stays the same. The vaccine has a purchase cost, a batch and an expiry, and its count must fall by one each time one is given. If the two are billed as a single line, the count on the shelf never matches the record. #### Keep vaccine stock by batch and expiry Enter each delivery of vaccine with its batch and expiry, and use the earliest expiry first. Look at what is about to expire at the start of each month, while there is still time to use it or return it. Store every vaccine as its manufacturer directs. Storage is outside the scope of this guide. #### Book the next visit before the family leaves Book the next vaccination or growth visit at the desk, on the day, from the date the doctor has set. Send a reminder before it. A family that leaves with only a date written on the card depends on someone at home reading the card in time. #### Keep well children apart from sick ones where you can If your space or your timings allow, give vaccination and growth visits their own hour or their own corner of the waiting room. A well baby who has come for a vaccine then does not sit beside children who have come with a fever. Keep them from blocking each other with four rules: sort each service by how long it takes and who does it, give hearing tests their own slots, keep the patient's token while they are away for a test, and bill the procedure before the patient leaves. The OPD stalls when the doctor leaves the consulting room for a twenty-minute procedure with a full room waiting, or when a patient sent for audiometry comes back to find their turn gone. #### Sort your services into three kinds List everything the clinic does and put each service in one of three groups. A clinic with an endoscopy set-up beside the consulting chair will sort differently from one with a separate procedure room. #### Give hearing tests their own slots Book audiometry by time, even if the OPD is walk-in. A hearing test needs a quiet room and a fixed length of time, and only one patient can be tested at once. Keep a few slots open in each session for patients the doctor sends across from that day's OPD, and book the rest ahead. If an audiologist visits on certain days, book test patients to those days and say so at the desk. #### Keep the token while the patient is away for a test A patient sent for a test should come back to the doctor without starting at the back of the line. Decide the rule and tell the desk: the patient keeps the same token, is marked as away, and is called next after the patient who is with the doctor when they return. Tell the patient this before they leave the consulting room, so they do not wait at the desk to be given a new number. #### Fix a block for longer procedures Choose when longer procedures happen and keep to it. Two patterns work: - A procedure block at a set time, such as the end of the morning OPD, with patients asked to wait or return for it. - One procedure after a set number of consultations, so the wait for the room rises in small steps. Urgent cases, such as a foreign body or a nosebleed, cannot wait for a block. Tell the waiting room plainly when one comes in. People accept a delay they can see the reason for. #### Get the report into the patient's file the same day File the audiogram, the tympanometry report and any endoscopy pictures on the day they are made. A hearing test is often compared with one from an earlier visit, and the comparison is possible only if both are in the file, dated, under the right patient. Name one person to do this at the end of each session. #### Record the side every time Write right, left or both on every ear and nose entry: on the order, on the procedure note, on the report and on the prescription. Make it a habit of the form, so that an entry without a side looks unfinished to whoever reads it. #### Bill the procedure before the patient leaves 1. Tell the patient the charge before the procedure or test, and take consent where the procedure needs it. 2. Add the procedure to the visit when it is done, in the room. 3. Make one bill at the desk for the consultation and everything added to the visit. 4. At the end of the day, compare the procedures recorded with the procedures billed. The doctor's minutes then go to the patient. None of this shortens the time a patient needs with you. It removes the minutes around it: finding the old record, taking the blood pressure, writing the same five medicines out again, and working out the bill. #### Where the time goes in a full OPD Watch one morning and note what the doctor does that someone else could do. The list is usually some of these: - Asking the patient's name, age and mobile number. - Looking for what was written at the last visit. - Taking weight, temperature and blood pressure. - Giving an injection or changing a dressing. - Writing out a prescription that is the same as the last ten for that complaint. - Telling the patient what to pay. #### Give a token the moment the patient arrives Register and give a token at the desk before anything else. A token settles the order, so nobody has to stand near the door to hold their place, and the desk can answer “how many before me?” truthfully. Take the name and mobile number once, at the first visit, and search for the patient after that. #### Take vitals before the patient reaches the doctor Have the sister or the compounder take weight, temperature, pulse and blood pressure while the patient waits, and enter them in the file. The doctor then starts with the figures on the screen, beside those of the last visit. For a patient who comes every month for blood pressure or sugar, the trend is the first thing the doctor needs, and it is ready. #### Send short tasks to the sister Keep injections, dressings, nebulisation and suture removal out of the doctor's line. A patient who has come only for a dressing or the next injection of a course does not need to wait behind twenty consultations. Decide which tasks the sister does on the doctor's standing instruction and which need the doctor to see the patient first, and write the rule down for the desk. #### Write common prescriptions once Save the groups of medicines you write together, and reuse them. A family doctor sees many of the same complaints, and the prescription for each is one you have written many times. Recall the set, then change it for the patient in front of you. Every prescription is still read and decided by the doctor: a saved set saves typing, and nothing else. #### Decide who is called out of turn Agree the rule before the rush and let the doctor make the call. A small baby, a frail elderly patient, a pregnant woman and anyone who looks seriously unwell should be marked at the desk, so that the mark shows in the doctor's line. The receptionist should not have to choose between patients in front of a full room. #### Bill at the desk, and close the day 1. The doctor adds what was done to the visit: the consultation and any procedure. 2. The desk makes the bill from the visit and takes payment by UPI, cash or card. 3. A part payment is recorded as such, with the balance left against the patient's name. 4.
